Description
the project aims to strengthen integrated and individualized community HIV prevention and treatment services for TB/HIV co-infection in Machakos County. To achieve the first outcome (Strengthened socio-health system for community HIV prevention and individualized care for people living with HIV), the following actions are proposed: a multi-level training program for healthcare personnel and community agents to enhance skills that facilitate service integration and individualized pathways; awareness campaigns to prevent HIV, TB, and STIs (Sexually Transmitted Infections) and address gender-based violence, encouraging youth involvement, particularly in strengthening Youth Friendly Centres: safe spaces for accessing information, services, and individual, group, and peer support. To achieve the second outcome (Strengthened integrated health services for the prevention of vertical transmission of HIV and TB/HIV co-infection), the following actions are proposed: supporting the National Mentor Mothers Program to facilitate women’s access to PMTCT services; intensifying efforts for early diagnosis and treatment of HIV/TB co-infection through systematic screening, utilizing Community Health Promoters; conducting operational research to analyze existing data and processes, identify areas for improvement, and propose innovative solutions to make services more effective, efficient, and integrated; and implementing integrated outreach initiatives at the community level. The initiative will be promoted through a partnership already established in Machakos: MMI, the Department of Diagnostics and Public Health at the University of Verona (UNIVR), and Deaf Empowerment Kenya (DEK), with the addition of the County Directorate of Health, Programs, and Partnerships Machakos, ensuring a systemic approach with impactful outcomes on the integrated services and the population accessing them. DEK’s involvement will foster youth engagement through its strong experience working with young people in the area; UNIVR will provide scientific expertise in service integration and monitoring system studies; MMI will contribute community work skills in employing community agents and reaching vulnerable populations that the healthcare system struggles to access. The project targets 7,045 individuals, divided into three age groups (10-19, 15-24, 25+), with a particular focus on individuals aged 10 to 24 to directly influence the triple threat.
