Description
The two-year project's overall objective is to contribute to the fight against HIV in remote communities in Mozambique and, as a specific objective, to introduce innovative strategies to improve the identification and linkage-to-care system of HIV-infected patients in remote communities in the Inhambane Province, while at the same time assessing its effectiveness through field research. The project aims to achieve the following results: R1. introduced and analysed innovative strategies to improve the cascade of care for HIV-infected patients; R2. developed innovative methodologies for the epidemiological characterisation of the rural resident population. The initiative intends to implement a community geolocation system and a qualitative survey on the demographic characteristics, social determinants and access to health services of the remote population, in order to: (i) develop effective strategies that increase the linkage to care of patients diagnosed and started on antiretroviral therapy within the strategy of Brigadas Móveis (mobile clinic service) and TARV Móveis (antiretroviral treatment service directly in the community); (ii) create and implement functional electronic tools for the purposes of data collection and analysis and the transmission of automatic alerts to patients. This will facilitate the acquisition and collection of SMART (Specific, Measurable, Achievable, Relevant, Timely) demographic and clinical data. The achievement of this outcome is a pivotal step in the pursuit of the goals of Agenda 95-95-95 on the HIV care continuum.The proposed activities refer to: research on HIV diagnosis pathways in paediatric patients at risk of vertical transmission in rural areas, through a mobile diagnostic tool (Point of Care Testing-POCT) for the determination of HIV RNA on plasma (HIV CV-PIMA) that can be used in brigadas moveis, for a rapid diagnosis of HIV and an early start of antiretroviral therapy (ART) in children (A.1 .1); determination by POCT of HIV plasma viral load for HIV-positive patients on therapy (A1.2); implementation and evaluation of innovative mobile health strategies aimed at increasing adherence to treatment and outpatient follow-up of patients with HIV (A1.3); creation and implementation of an electronic system for collecting clinical data from patients by health workers in mobile clinics (Brigadas Móveis) and decentralised HIV care clinics (Tarv Móveis) (A2. 1); introduction of electronic geolocalisation systems, for the collection of community demographic data that will make it possible to identify the characteristics of the population (age, sex, household composition), knowledge of the epidemiological aspect and consequent identification of targeted health strategies. A qualitative survey will be carried out to analyse the social determinants and the main challenges to access to health services perceived by the communities served (A2.2); sharing the results obtained with local and international realities through seminars, workshops and the publication of scientific articles (A2.3).
