Description
The project will respond to the critical issues faced by the prison population, which represents one of the key and vulnerable populations (KVPs) at greatest risk. Identified needs include limited access to prevention, screening, diagnosis, and care services for prisoners with TB-HIV, as well as integrated psycho-social support services and community and professional reintegration; human rights and gender-based barriers to access to health services; specific gender barriers and disproportionate needs of women, girls and other minorities. The strategy contributes to ending the TB and HIV epidemics (G.O.) through actions aimed at strengthening equity in access to integrated early diagnosis, prevention and treatment services for TB and HIV, regardless of gender, social status or other human rights based barriers (S.O.). The objectives will be achieved through the following 2 components: (R1) Awareness-raising and advocacy interventions aimed at removing systemic and human rights-based barriers that hinder access to health services for people with TB and/or HIV living within the Kenyan prison system. The component includes research activities conducted by the UPO (KAP study on new TB screening and diagnosis practices, pilot study on the innovative and approved HIV rapid tests, final impact assessment study); awareness-raising activities addressed to senior officials of KPS and MoH, KPS custodial and clinical staff, and the inmate population, on the relationships between stigma and discrimination, right to health and access to care for HIV and TB infections; advocacy actions (thematic working group, final conference, protocol reviews and production of policy briefs). Awareness-raising activities will combine peer-to-peer approaches (through TB and HIV Peer Champions, paralegals), training led by project staff and awareness-raising events for the prison community. (R2) Interventions to strengthen integrated personal services, such as TB and HIV health and prevention services, psycho-social support and rehabilitation, and professional, family and community reintegration in the post-release phase. The strengthening of integrated services, aimed at protecting multiple vulnerabilities resulting from living with the infection and from the state of detention, will take place through the following activities: training interventions for KPS staff (clinical and non-clinical) on diagnosis and treatment, psycho-social support and community reintegration through direct trainings and ongoing mentoring (OJT); infrastructural rehabilitation and equipping of outpatient clinics and laboratories; nutrition and immune function support; and individualized interventions for psychological support and for occupational, family and social reintegration of TB-HIV+ prisoners.
