Description
The initiative has been developed by the NGO Doctors with Africa CUAMM (CUAMM) jointly with the Dipartimento di Medicina Sperimentale e Clinica, Università degli Studi di Firenze (DMSC), l’Istituto Superiore di Sanità (ISS)- Centro Nazionale per la Salute Globale (CNSG) in close collaboration with two local partners, YAWE – Youth And Women Emancipation and NACOPHA – The National Council of People Living with HIV/AIDS. The project, which aims "To end AIDS, Tuberculosis and Malaria" (General Objective), aligns with the priorities of the Tanzanian health authorities in achieving SDG 3.3. and the strategic objectives a, b and c of the Global Fund. The initiative promotes a participatory and people-centred approach aimed at improving quality of prevention, care, treatment and follow-up services in the Tanzanian context, characterized by a young population highly exposed to the risk of contracting HIV. The Shinyanga Region of Tanzania has a population of 2.241.299 people of which 35% are adolescents and young adults aged between 10 and 24 years. The prevalence of HIV in the population among adults 15-49 years is higher than national level, 5.9% (source: THIS 2016/17), with a significant gender disparity (7.2% among women while 4.3% among men). In the Shinyanga region, the % of women who have never been tested for HIV is the highest at national level, equal to 34.4% (TDHS 2022), while the percentage of women aged 15-24 with knowledge about HIV is limited to 25.7% (TDHS 2022). Since 2015, CUAMM has been operating in two districts of the Shinyanga Region, Shinyanga Municipal Council and Shinyanga District Council (total population 684,561, of which 231,397 in the 10-24 age group) and several initiatives have been implemented over the previous years in support of HIV. In particular, within the Global Fund framework, between April 2022 and September 2023, CUAMM in collaboration with the University of Florence implemented the project "Improving the health and well-being of HIV+ adolescents and young adults, AID 12349/01 /6” in the two districts mentioned above. The operational research conducted during the previous intervention highlighted the problems and challenges faced by adolescents and young people in Shinyanga, which constitutes the basis of this proposal. Specifically, in the target health centers and communities, the following problems were detected in relation to the population group aged between 10 and 24 years: Limited access to prevention and testing services especially among young people and adolescents: there is a high percentage of young people between 15 and 24 who have never been tested, especially young women and girls. Limited quality of care, treatment and follow-up services within the CTCs including i) Lack of adequate trained human resources, ii) poor differentiation of services for young people and adolescents, iii) Difficulties in ensuring adherence and follow-up -up of HIV-positive patients, v) Partial effectiveness and efficiency of the supply system Lack of an inclusive community environment and presence of discrimination and stigma towards HIV-positive people: Stigma and discrimination continue to hamper the success of HIV programs in Tanzania, with a Stigma Index of 13.7% (2020). This initiative aims to strengthen prevention and testing services at both health facilities (CTCs) and community level. Furthermore, the action will improve service delivery through the adoption of a "people-centered" approach that encourages the differentiation of services, placing more emphasis on the psychological and counseling component for people with HIV.
