| vocab:Description | A.S.S.I.S.T. – Accesso a un Sistema Sanitario Integrato per i Servizi per TB/HIV in Guinea è un’iniziativa biennale promossa da ACAP, in partenariato con l’Università degli Studi di Roma Tor Vergata (TVG) ed il centro di salute locale Dispensaire Maternité Saint Gabriel (DSG). L’obiettivo è Ridurre la mortalità e l’incidenza di HIV attraverso l’accesso universale alla prevenzione e al trattamento HIV/TB in Guinea, aumentando l’accesso della popolazione di Conakry, Coyah e Dubréka ai servizi sanitari per HIV/TB gratuiti e di qualità. L’analisi dei bisogni, condotta da un team di esperti, ha evidenziato la necessità di intervenire per il rafforzamento del sistema sanitario su 3 problemi: • Inaccessibilità geografica ed economica dei siti diagnostici e di trattamento per HIV/TB, soprattutto nei contesti periferici, la quale porta spesso ad una diagnosi tardiva nei pazienti e, quindi, contribuisce all’elevata mortalità dei pazienti affetti da HIV/TB; • Scarsità di strutture adeguate, farmaci e reagenti di laboratorio per HIV/TB, che non permette un’equa distribuzione dei servizi sanitari per HIV/TB sul territorio, impattando fortemente sull’accesso alle cure; • L’elevata perdita di pazienti nel circuito di diagnosi e trattamento, poiché anche molti dei centri più equipaggiati a livello nazionale, spesso, non riescono a garantire la continuità del servizio, interrompendo la treatment cascade. A partire dai suddetti problemi, attraverso l’esperienza e le collaborazioni messe in atto negli anni, ACAP ha individuato i seguenti bisogni, sui quali intende incidere con la presente Iniziativa: • Incrementare l’accesso ai siti diagnostici e di trattamento per HIV/TB nelle zone periferiche: per venire incontro ai bisogni dei pazienti che lamentano il costo elevato dei trasporti per recarsi nel centro di Conakry, si offrirà un pacchetto di cure e analisi gratuite. Inoltre, si potenzieranno i servizi offerti nelle aree periferiche di Conakry (Matoto) e nelle zone rurali di Dubréka e Coyah per ottenere una maggiore decentralizzazione mantenendo alta la qualità del trattamento, attraverso l’instaurazione di un sistema inter-ospedaliero coordinato di tipo “Hub & Spoke” (H&S) per la gestione integrata dei servizi per HIV/TB. • Riduzione della mortalità tra le persone con HIV/TB: s’intende rispondere a questo bisogno promuovendo una maggiore integrazione dei servizi HIV/TB. Oltre all’hub del Centro di Dixinn, quindi, la ricerca attiva e la diagnosi molecolare della TB sarà estesa anche agli spokes periferici di Dubréka, Coyah e Matoto. • Miglioramento delle performance della Prevention of mother-to-child transmission (PMTCT): Per migliorare della PMTCT ACAP potenzierà il counseling alle donne in gravidanza attraverso una formazione ad agenti comunitari che sarà aperta anche ad operatori di altre strutture e aumenterà la diagnosi precoce nei bambini mettendo a disposizione il laboratorio Centro di Dixinn e installando appositamente un GeneXpert nel DSG. • Aumentare il testing HIV/TB e l’aderenza dei pazienti: saranno formati nuovi counsellors e attivati dei front-desk nei 3 centri periferici per aumentare il testing alle differenti porte di entrata. Inoltre, sarà rafforzata la retention attraverso migliori strategie di comunicazione e referral, e rafforzando il supporto psicosociale e l’educazione terapeutica. Infine, sarà condotta una specifica ricerca al fine di identificare le ragioni dell’insufficiente retention e le principali barriere all’accesso ai servizi di testing, PMTCT e cura per HIV/TB nei contesti periferici, sia urbani che rurali. |

The need-assessment highlighted 3 main problems: • Geographical and economic inaccessibility of HIV / TB diagnostic and treatment services; • Scarcity of adequate facilities, drugs and laboratory reagents for HIV / TB; • The high loss of patients in the diagnosis and treatment flow; Starting from the aforementioned problems, ACAP has identified the following needs, on which it will act: • Increase access to HIV / TB diagnostic and treatment services in peripheral areas: the services offered in the peripheral areas of Conakry and in the rural areas of Dubréka and Coyah will be enhanced to achieve greater decentralization while maintaining high quality of treatment, establishing a “Hub null (Hnull) system for the integrated management of HIV / TB services. • Reduction of mortality among people with HIV / TB: In addition to the Dixinn Center’s hub, active research and molecular diagnosis of TB will also be extended to the peripheral spokes of Dubréka, Coyah and Matoto. • Improvement of the performance of the Prevention of mother-to-child transmission (PMTCT): counseling for pregnant women will be enhanced and early infant diagnosis will be increased by installing a GeneXpert. • Increase HIV / TB testing and patient adherence: new counselors will be trained and front-desks will be activated in the 3 peripheral centers to increase testing at the different doors of entry. In addition, a specific research will be conducted in order to identify the reasons for insufficient retention and the main barriers to access to testing services, PMTCT and treatment for HIV / TB in peripheral contexts, both urban and rural. |
